Abstract
Tujuan Penulisan Laporan Akhir ini adalah untuk membangun Aplikasi Klasifikasi Pasien Berbasis Website yang meliputi pendaftaran dan menentukan klasifikasi pasien rehabilitasi. Metode yang digunakan untuk pembuatan laporan ini adalah menggunakan metode Waterfall. Proses yang digunakan untuk mendesain sistem ini adalah Unified Modeling Language (UML) dan menggunakan Bahasa pemrograman PHP berbasis Framework Laravel serta database MySQL. Sistem ini terbagi menjadi 2 bagian yaitu untuk Admin dan Pasien. Dengan dibangunya sistem informasi ini diharapkan dapat membantu bagi calon pasien pecandu narkotika yang akan melaksanakan rehabilitasi dengan mudah, efektif dan efisien dan meminimalisir kesalahan yang terjadi.
