Abstract

Rekam medis merupakan catatan riwayat medis pasien, setiap dokter atau dokter gigi dalam menjalankan praktek kedokteran wajib membuat rekam medis, namun diketahui bahwa di Klinik Jurusan Kesehatan Gigi Politeknik kesehatan kementerian kesehatan Palembang pendataan rekam rekam medis gigi masih dilakukan secara manual, sehingga mempengaruhi efisiensi dan efektivitas dalam pengelolaannya. Maka dari itu tujuan penelitian ini adalah untuk membangun sebuah Aplikasi Rekam Medis di Klinik Jurusan Kesehatan Gigi Politeknik Kesehatan Kementerian Kesehatan Palembang dengan dengan menggunakan framework Laravel, pemrograman PHP dan MySQL sebagai database-nya. Metode yang digunakan yaitu waterfall, dari proses analisa kebutuhan, kemudian perancangan sistem menggunakan UML, implementasi program sampai dengan pengujian. Aplikasi ini berisi menu dashboard, pasien, treatment, rekam medis, pengguna, dan pengatuan. Dengan dibuatnya aplikasi ini diharapkan dapat mempermudah pendaftaran serta pendataan rekam medis pasien yang terkomputerisasi.
