Abstract
The aim of this final report is to fix the existing problems in the labor unit of human resources (HR) that is a data processing system performance appraisal is still manual processing. There are several weaknesses including work unit superiors to assess their employees fill out a questionnaire form that has been given by part of the working unit of human resources (HR), then if the employee performance appraisal form has been filled by a superior work unit, the form is returned to the working unit of human resources (HR), then part of the working unit of human resources (HR) collecting questionnaire which has been filled by the work unit supervisor for input into Microsoft Excel. So that in the process less efficient. To realize these solutions, it needs an application that can generate rapid process making it easier for admins (SDM) in data processing. The benefits in this application is to make easy for admin (SDM) in data processing performance appraisal. Data were collected by direct observation of the activities - learning activities related to informatics. The results obtained, namely the Data Processing Applications Performance Assessment Employee at PT Bukit Asam (Persero) Tbk. The conclusion in this application is the application of data processing performance appraisal using the programming language PHP and MySQL that can be accessed by admin (HR), and the work unit supervisor.
