Abstract
Penelitian ini bertujuan untuk membuat suatu sistem untuk menghitung hasil evaluasi pelayanan Puskesmas berbasis website yang dapat membantu proses perhitungan penilaian hasil penilaian evaluasi Puskesmas. Hasil dari tugas akhir ini adalah implementasi metode Fuzzy Logic Tsukamoto pada Aplikasi Evaluasi Pelayanan Puskesmas Berbasis Website dengan Metode Fuzzy Logic Tsukamoto pada Dinas Kesehatan Kota Palembang yang memberikan kemudahan untuk pegawai Dinas Kesehatan Kota Palembang khususnya bidang Pelayanan Kesehatan dalam menghitung hasil evaluasi pelayanan puskesmas yang telah mereka survei sebelumnya dan pihak Dinas Kesehatan Kota Palembang dapat mengambil tindakan dari laporan yang dihasilkan. Aplikasi ini dapat menjadi acuan untuk kedepannya ketika akan dilakukannya pengevaluasian kembali.
